The Lantronix serial ports are 6-wire 6-position RJ11 jacks which meet the APO 1994 serial standard. Hence they can be used directly. If preferable, one can use an octopus to connect the terminal server to a patch panel or harmonica.
The octopus required is a standard item: a USOC-wired octopus with a female Telco connector and 8 legs. Each leg has an RJ11 plug. Buy a short one with NON-twisted pair legs; if the legs have twisted pairs it is guranteed that the wrong wires will be twisted together. Remember: be sure that the octopus is USOC-wired; there are other incompatible standards.
On 2/4/94 Data Comm Warehouse (800-328-2261) sold these octopi as item DCA1138 for $21.
If a custom part must be made, here are the specifications and derivation.
Each octopus consists of:
Each leg is four feet (4') long unless otherwise specified. The wiring is as follows. Use of twisted pair wire is optional, but if it is used, it must be used as shown in the table.

RJ-45 pins 2 and 7 must not be connected to anything. Telco pins 25 and 50 are also not connected.
The octopus wiring is derived from the table below, as follows:
