SECTION 28. CONFINED SPACE

28.1 INTRODUCTION

APO has identified two permit-required confined space areas and, consequently, developed a plan for dealing with these through education and control. These areas are the cones of the 2.5-m and 3.5-m telescopes and are appropriately signed. These cones have sloping walls that converge to a tight space around the main azimuth bearings. While the areas have platforms in them for working, an employee may slip and become entrapped. In addition, there is compressed Nitrogen gas going through these areas which, if a major leak developed, could reduce the Oxygen content of this small area even though there are ventilation holes in the cone's wall.

This section will cover both employees and visitors. Visitors who require access to a confined space must either provide proof of outside training or attend a training session.

28.2 CONTROL

The access hatches are to be locked at all times except during periods of maintenance per this section. The keys for these areas will be held by the entry supervisors, the keys will not be kept in public domain. The entry supervisors will be the telescope engineers for the respective telescopes and the APO Site Safety Officer. These supervisors will be responsible for controlling access to these areas.

Entry permits will be completed by each supervisor when necessary. These permits will be posted on the door of the respective telescopes so that anyone entering the area will be aware that confined space work is in progress. After cancellation the permits will be archived by the Site APO Site Safety Officer.

Atmospheric testing must be accomplished prior to and during entry. Due to the nitrogen gas oxygen concentration level must be monitored and recorded. In addition the telex exhaust fans should be activated during entry do remove any buildup of nitrogen that could occur and ensure a fresh air supply for workers. Additionally, if flammable solvents are used, Flammable (LEL) monitoring is required.

Chlorinated solvents shall be not be used in these areas.

Prior to entry the entry supervisor shall notify the Sunspot fire department the confined space is being entered. They shall also notify them once the permit is cancelled and the area is secured.

28.3 TRAINING

The training program is one prepared by J.J.Keller & Associates and obtained through Emedco. This program will be used to train entrants, attendants, and supervisors. The Sunspot Volunteer Fire Department will provide rescue services. The local fire department will be provided information and given briefings on the status and use of these confined spaces. Training will be provided to the fire department upon request. The APO Site Safety Officer shall establish the course outline and conduct the class that will consist of the training material previously mentioned and the site specifics. The APO Site Safety Officer shall maintain the training records and material.

All persons requiring access to these spaces for work must receive training as an entrant and an attendant. Certain persons may only require attendant training. Supervisors shall be trained as an entrant, attendant and as a supervisor. Persons receiving this training shall be given a test to ensure their understanding of the requirements. A 70% is required for passing.

New employees will be trained, as their job functions require it. Retraining will take place as necessary when regulations are changed, new hazards are added, or an employee shows a deficiency in training.
