Please note that this form can also be used to order back issues of the journal (subject to availability) by indicating the beginning and end of the subscription. However you are advised to contact the Editorial Office regarding individual back issues and before making a request using this form. The Editorial Office will advise you of the best procedure to follow to secure back issues and provide you with current prices.

This page tells you exactly what is required on the subscription form and why.

1. Name - This should be your full name if you are making a private subscription or the name of the department of your institution. This will be the first line of the address for dispatch of journals and the subscriber name to which we refer in correspondence.
2. Address - This should be the full address to which journals should be sent including region or state, zip or postal code and country. Please note that a maximum of six lines are permitted for the full address. This is also the address to which we may send reminders or other correspondence concerning your subscription.
3. Contact - If the address provided is different to that to which administrative matters should be sent please indicate here the address which should be used for official business. Alternatively you may provide us with the name and address of your agent.
4. Email - This is an email address at which we may contact you if we need to do so urgently. We will also send an acknowledgement of your subscription request to this email address. Please leave this space blank if you do not wish to be contacted by email.
5. WWW - If you provide any WWW services you wish us to know about and possibly include on our server please give the URL here.
6. Number - This should indicate the number of subscriptions you wish to make under the same Name and Address. For example if you wish two copies of each issue you should place a 2 here.
7. Start - This should indicate when you wish the subscription to commence. For example you may wish to subscribe from 1995 onwards in which case you will be invoiced for all back issues from the beginning of 1995 as well as for the subscription rate for the present year. If you are making a subscription through an agent please be sure to explain when the subscription is to begin. If you wish the subscription to begin in the present year only simply type the current year in this space. You may also indicate the start of the subscription by the relevant volume and issue number, e.g. Volume 22 number 1.
8. Stop - If you wish the subscription to run only until the end of the present year please leave this space blank. We do not at present accept payment in advance for subscriptions beyond the current year. You will receive a reminder of the subscription rates and renewal policy in October each year unless you cancel your subscription. You may indicate here when you wish the subscription to stop (by year or volume and issue number) if you are ordering back issues only.
9. Type - Please indicate here the type of subscription you are making. If you are a private individual or represent an official society you should contact the Editorial Office prior to submitting this form.
10. Payment - Please indicate the type of payment we should expect. If you indicate Invoice you will be invoiced directly for your requested subscription. If you indicate Agency we will take no further action and await notification from your agency.
