This Web Site is designed to provide periodic updates to the material covered in the book. In most cases these updates are based on new data that has come out after publication. In some cases, sections have been updated to better reflect the proper historical sequence of investigation of that topic. Hopefully these updates will keep the book relevant as it lays down the foundation to be supplemented by electronic plug-ins based on new data. Without the book , however, this site will not be very useful.
